As part of regulatory requirements and to ensure that we keep you connected to what is happening with your account, you are required to submit your up-to-date information to us, even if your information has not changed within the last year, as we must also report on the recency of your information.

We are also introducing our Mastercard ® debit card soon which will be distributed to eligible members by postal mail.

To satisfy this requirement and to ensure that you receive your card, you are required to update your account information by submitting the following documentation to us by Thursday, April 15th, 2021:

  • Completed General Information Form
  • Two (2) forms of valid government-issued photo identification (National I.D Card, Driver’ License or Passport)
  • National Insurance Card
  • Proof of Address dated no older than three months to the date submitted (E.g. Utility Bill, Credit Card Statement, or Bank Statement bearing your name and residential address). – Cell phone bills are NOT accepted.
